Flask 1.0 does not support the flask.ext imports that this project currently uses.

If the app is not initialized in the
Autodocobject, either through__init__orinit_app, calls todoc()seem to work correctly, there is a fairly cryptic error when callinghtml().Explicitly raise an exception if
self.appis not defined here, to make it easier to debug this problem.
